My child has my permission to attend the Atascocita High School Future Patriette Clinic on September 26, 2026, and dance during halftime at the game October 3rd, 2026. I understand that the Humble Independent School District, the instructors, sponsors, teachers, and booster parents shall NOT be held responsible for any expenses incurred in the emergency treatment of my child. They are also NOT liable for any accidents, injuries, theft and/or damage to person or property during the duration of the clinic.

I hereby give the Patriette Booster Club, representatives, and authorized media organizations permission to print, photograph, and record my child for use in audio, video, film, or any other electronic, digital and printed media.This is with the understanding that neither the Patriette Booster Club nor its representatives will reproduce said photograph, interview, or likeness for any commercial value or receive monetary gain for use of any reproduction/broadcast of said photograph or likeness.I am also fully aware that I will not receive monetary compensation for my child’s participation.
